Reimbursement

The costs of all invited participants will be covered by a grant from the Division of Integrative Biology and Neuroscience at the National Science Foundation. All reimbursements will be processed through Utah State University. Participants should send their travel information to us in advance to facilitate reimbursement.

What Will be Covered

Allowable costs for participants in the workshop are: If you have some alternative travel arrangement that you would like to organize (e.g. driving to USU with your own car) please contact Jim Powell regarding reimbursement.

Participants need to send their travel information to Alison Pence as soon as possible. Alison is the staff assistant designated to facilitate this workshop; she may be contacted at (435) 797-2595 or by email at assist@math.usu.edu.

Information for Reimbursement

The following information must be sent by email to Alison Pence so that she can file a travel authorization (TA) for participants. This TA is the mechanism by which participants are reimbursed, and with a TA on file the traveller receives additional insurance coverage through Utah State University. Therefore, for safety and also to speed up reimbursement, please send Alison the following information ASAP:
  1. Complete name
  2. Social Security Number
  3. Institutional affiliation and mailing address
  4. Dates of Travel
  5. Purpose: mention that you are a participant in the ``Systems Biology Workshop''
  6. Contact information: phone and email
  7. Total cost of your flight to and from SLC airport
  8. Flight information: airline, when you are leaving, when you are arriving
  9. Any expenses (shuttle, parking, mileage) required to get to the airport on the `home' end of your travel. If you must drive a significant distance to the airport, please estimate the roundtrip mileage.
  10. Number of days you will be staying at the University Inn here on campus.
The more exact the numbers provided above, the better, but please provide estimates even if you don't know the exact number.
